Manufacturers
If you are applying for an agent's licence along with a manufacturer's licence, you will need a manufacturer's application package. You'll find the application package containing all the forms you need for both licences on the web page called Submitting your Manufacturer’s licence application These packages are also available from Liquor Control and Licensing Branch Head Office (call toll-free 1-866-209-2111).
Independent agents
If you are applying as an independent agent, you will need to complete the agent's application package, available by mail from the Liquor Control and Licensing Branch Head Office (call toll-free, 1-866-209-2111), or you can print each of the individual pieces from this website. The package contains:
- Application for an Agent's Licence (LCLB034)
- Required forms for a criminal record search (you or your resident manager and any shareholders and partners are required to complete the following criminal record search forms):
